Dear Colleagues of Digital Square,

The final deadline to submit Notice D concept notes to the OAP platform is tomorrow, May 31, 2019. Please note that applicants must use the concept note template, which is new this year. Please ensure that your concept note, uploaded in the supporting documents section, meets the following requirements:

1. 3 pages or less
2. Created directly in the concept note template
3. Contains the following elements:
   1. Title
   2. Two Sentence Overview
   3. Executive Summary
   4. Consortium Team
   5. Project Description

In addition, on the OAP platform, please ensure you have:

1. Copied the first three sections of the concept note into the OAP platform application page and complete the consortium team field
2. Provided Digital Health Atlas registration confirmation
3. Provided tagging, geographic reach, and source code, as applicable

Digital Square will notify applicants of in-scope concepts on Monday, June 10, 2019.
