We hope you are doing well. Digital Square, a PATH-hosted initiative, connects health leaders with the resources necessary for digital transformation. In an effort to improve Digital Square procurement processes to make them more equitable and accessible, we are seeking feedback from organizations who created an account on Digital Square’s open application platform. We are particularly interested to hear from organizations that might be hesitant to respond to future Digital Square Notices or other solicitations.

The questionnaire, available here, should take about 5-10 minutes to complete. All questions are optional unless indicated with an asterisk. Your responses will be confidential and anonymous unless you provide your name. You will have the option to indicate if you are interested in potentially speaking with us in a one-hour virtual interview to expand upon your feedback.

Digital Square will publish findings from this questionnaire and subsequent interviews in a blog post in early 2023. If any data are to be published, your responses will be combined with those of other respondents and presented in an aggregated or anonymized form. Participating (or not participating) in this questionnaire will have no bearing on future funding decisions.
